Getting Started with OPWDD

Starting the OPWDD process can feel overwhelming, but breaking it down into clear steps makes it much more manageable. Here is an overview of the key milestones on the path to OPWDD services and Self-Direction.

Step 1: Determine Eligibility

The first step is to contact your local OPWDD Developmental Disabilities Regional Office (DDRO) to begin the eligibility determination process. You will need to provide documentation of the developmental disability, including medical records, psychological evaluations, and school records if applicable.

Step 2: Complete the Intake Process

Once eligibility is confirmed, OPWDD will conduct an intake assessment to evaluate the individual's support needs. This assessment helps determine the level of funding and services the individual is entitled to receive.

Step 3: Develop a Life Plan

Every OPWDD participant has a Life Plan — a person-centered document that outlines their goals, strengths, preferences, and the supports they need. The Life Plan is developed collaboratively with the individual, their family, their Care Manager, and other key people in their life.

Step 4: Choose Your Service Model

OPWDD offers both traditional agency-based services and Self-Direction. If Self-Direction is the right fit, you will work with a Support Broker to develop a spending plan and begin building your support team.

Step 5: Connect with a Support Broker
